New Patient Forms and HIPAA Notice of Privacy Practices
For your convenience, each of the forms below may be reviewed and printed for you to fill out in advance of your first visit from any computer with printer capabilities.
The Patient Registration Form, the Medical History, and the Authorization For Release of Identifying Health Information are all available for printing. Please print each of them, fill them in, sign where indicated, and bring them along on your appointment day to help expedite the New Patient First Visit.
If you have an extensive list with medication and dosage information, please feel free to bring that along and will make a copy to attach to your Medical History.
The HIPAA Notice of Privacy Practices is for your information only. You need not print it or bring it with you.
